Freedomball Soccer Program Liability Waiver:

I acknowledge that neither my minor child nor I will file claims, lawsuits, or take any legal action against Freedomball Soccer Program, its agents, sponsors, contractors, suppliers, or employees for any injuries, damages, or losses - whether personal or property-related - that may arise from my child's participation in these sporting activities. This waiver releases Freedomball Soccer Program and all associated parties from liability, including negligence, in relation to my child's involvement in the league, programs, camps, clinics, or any related activities, except in cases of willful or reckless misconduct.

I UNDERSTAND THAT SPORTS CARRY THE RISK OF PHYSICAL CONTACT, AND THAT SERIOUS INJURIES (INCLUDING DEATH) OR PROPERTY DAMAGE MAY OCCUR DURING THESE ACTIVITIES. I AM FULLY AWARE OF THESE RISKS, I VOLUNTARILY ASSUME THEM ON BEHALF OF MY MINOR CHILD AND RELEASE FREEDOMBALL SOCCER PROGRAM, ITS AGENTS, SPONSORS, AND EMPLOYEES FROM LIABILITY ARISING OUT OF NEGLIGENCE OR CARELESSNESS. I affirm that I am at least 19 years old and that my child is in good physical health, with no medical conditions that would prevent participation. We agree to adhere to all rules and guidelines governing the league, camp, or clinic. Additionally, we take full responsibility for the condition and maintenance of any sports equipment used, whether owned personally or provided by Freedomball Soccer Program or its affiliates.

BY SIGNING THIS AGREEMENT, I CONFIRM THAT I HAVE READ AND UNDERSTOOD ITS TERMS. I RECOGNIZE THAT THIS IS A LEGALLY BINDING RELEASE OF LIABILITY FOR BOTH MYSELF AND MY CHILD, ENTERED INTO FREELY AND VOLUNTARILY.

Internet & Social Media - Agreement & Expectations

This Internet & Social Media Agreement sets the standards for all participants in the Freedomball Soccer Program, including players, parents, coaches, staff, and club directors. Participation in the program is a privilege, and all members are expected to maintain professionalism both offline and online, adhering to the Freedomball Soccer Program code of conduct.

This agreement applies to the use of personal mobile devices and computers during active engagement with the Freedomball Soccer Program.

PLAYERS: While this section primarily addresses players, the guidelines also apply to all participants, including club directors, parents, coaches, and staff.

• I recognize that the privilege of using the internet and social media while representing Freedomball Soccer Program comes with responsibility. I will refrain from using my digital device during games or practices unless granted permission by staff.
• I will not record, photograph, or live stream other players, coaches, or staff without their explicit consent.
• I am fully responsible for my personal digital devices and acknowledge that Freedomball Soccer Program is not liable for any loss or damage to them.
• I will not share personal information of any program member - including names, addresses, and contact details - without their consent.
• I will not engage in online bullying, harassment, or any inappropriate digital behavior towards any member of the Freedomball Soccer Program.
• I will avoid using my digital device in ways that are considered rude, inappropriate, or disruptive by others in the program.
• I will report any instances of online bullying, harassment, or inappropriate messages to my coach, manager, or program staff.
• I will ensure that my online behavior, including posts and comments, aligns with the values of the Freedomball Soccer Program, refraining from profanity or inappropriate content.
• As long as I am affiliated with the Freedomball Soccer Program, this agreement applies both on and off the field whenever using the internet, social media, or any digital device.
• I understand that violating these guidelines may lead to disciplinary actions, including loss of privileges, suspension, or removal from the program.

Freedomball Soccer Program Staff (including coaches, managers, contractors, and employees):

• We recognize that players may have reasonable explanations for certain actions. If the explanation is valid, no punitive action will be taken.
• We commit to setting a positive example regarding internet and social media use.
• We understand that players may make honest mistakes. If a player reports a mistake voluntarily, We will support them rather than punish them. However, if misconduct is intentional or concealed, appropriate disciplinary actions will be taken according to Freedomball Soccer Program policies.

Freedomball Soccer Program Players & Staff - Social Media Waiver & Release:

• I hereby give Freedomball Soccer Program permission to use photographs and video recordings of me for promotional, advertising, and media purposes, including publication in any format, without any restrictions. I release Freedomball Soccer Program and its representatives from any claims or liabilities arising from the use of these images or videos. Additionally, I consent to the use of my statements from interviews, with or without my name, for advertising and publicity purposes, without any entitlement to compensation.
